PHYSICIAN - PSYCHIATRIST: AURORA, BENSENVILLE, ELGIN - FULL-TIME - HEALTH CENTER

Education: Doctor of Medicine or Osteopathic Medicine degree with a valid license to practice psychiatry in the State of Illinois. Board certification or board eligibility in the field of psychiatry.

Knowledge/Skills: Possess specific knowledge of behavioral medicine health care. Health care clinic setting a plus. Excellent interpersonal skills to receive and convene with patients, families, staff and other physicians possessing or describing a variety of concerns. Demonstrate in all duties and responsibilities a warm, humane, compassionate attitude toward patients, their families, visitors, customers, and employees of VNA.

Type/Years Experience: Minimum of five years of experience practicing medicine. Two to three years of delivering medical leadership preferred.

Job Summary: Evaluates patients for mental, emotional or behavioral disorders. Diagnoses disorders, including nature and extent, and determines the appropriate course of treatment for the patient. Prescribes medication when necessary. Requires a degree in medicine from an accredited school and is licensed to practice.

PHYSICIAN - FAMILY PRACTICE - HOUSE CALLS:  AURORA - PART-TIME

Education: Board Certified Physician in Family Practice preferred. Licensed in Illinois required.

Knowledge/Skills: Must have the knowledge, skills and ability to provide care to patients. Must be able to be on rotation to take call.

Type/Years Experience: 1-2 years experience preferred as a Family Practice Physician.

Job Summary: The family practice physician provides health assessment and direct care to patients as part of the provider team providing services to VNA House Calls patients.

PHYSICIAN - PODIATRIST:  AURORA - PART-TIME - HEALTH CENTER

Education: Board Certified Physician in Podiatry. Licensed in Illinois required.

Knowledge/Skills: Must have the knowledge, skills and ability to provide care to patients. Must be able to be on rotation for hospital rounds and take call.

Type/Years Experience: Training or experience in Geriatric Care, certification a plus.

Job Summary: The Podiatrist provides health assessment and direct care to patients in both our Health Center and off-site locations to patients as part of the provider team.

PHYSICIAN - PEDIATRICIAN:  ELGIN - FULL-TIME - HEALTH CENTER

Education: Board Certified Physician in Pediatrics preferred. Licensed in Illinois required.

Knowledge/Skills:  Must have the knowledge, skills, and ability to provide care to pediatric patients. Must be able to be on rotation for hospital rounds and take call.

Type/Years Experience: 1-2 years experience preferred as a Pediatrician.

Job Summary: Provides primary care at our Federally Qualified Health Centers. Provide comprehensive medical evaluation and management of clients. Work with other physicians focused on family practice, obstetrics, and gynecology, as well as our nurse practitioners.

PHYSICIAN - OB/GYN:  ELGIN - FULL-TIME - HEALTH CENTER

Education: Board Eligible or Board Certified Physician in OB/GYN preferred. Licensed in Illinois required.

Knowledge/Skills:  Must have the knowledge, skills and ability to provide care to obstetrical patients. Must be able to be on rotation for hospital rounds and take call.

Type/Years Experience: 1-2 years experience as an Obstetrician preferred.

Job Summary: Provides primary care at our Federally Qualified Health Center. Provide comprehensive medical evaluation and management of clients. Work with other providers focused on family practice and pediatrics.

NURSE PRACTITIONER - FAMILY PRACTICE:  CAROL STREAM/BENSENVILLE - FULL-TIME - HEALTH CENTER

Candidate must be willing to work a schedule at our Carol Stream and Bensenville clinic sites as needed.

Education: Graduate of accredited advanced practice program with certification as Family Nurse Practitioner, Illinois advanced practice license.

Knowledge/Skills: RN/NP skills necessary to perform physical exams.  Bilingual in Spanish/English preferred

Type/Years Experience: Greater than 1 year primary care experience for men, women and children.

Job Summary: Under collaborative agreement with a VNA Health Center physician, provides comprehensive medical evaluation, education and management for clients throughout the life cycle that present for routine preventive, acute and chronic disease health services at the VNA Health Center clinic sites (on and off site clinics).

EHR CLINICAL COACH:  AURORA - FULL-TIME

Education: Bachelor’s degree in nursing preferred.

Knowledge/Skills:  Registered Nurse in the State of Illinois required.  Clinical workflow and an understanding of clinical terminology required.  Superior communication and public speaking skills required.

Type/Years Experience: Three years of experience working directly with providers and/or nursing staff and patient care charting practices. PC/Software experience to include Microsoft Office (Word, Excel, Outlook, PowerPoint).  Experience with classroom style training.  Two years of experience in the area of software training, preferably in a healthcare setting.  EHR implementation and training experience preferred. 

Job Summary: The EHR Clinical Coach facilitates the development and preparation of training materials, training schedules, and conducts training for providers and staff members, as evidenced by successful departmental use of the EHR.  This position provides training on the Electronic Medical Record (EMR) systems, for all new hires and existing staff members, with an emphasis on provider staff.  The position contributes to the development of best practices and workflow.  Stays current on system changes and updates training sessions and materials as needed.  Works closely with the IT Team on system related issues and performs special projects as needed.  This position is an integral figure in implementing, maintaining and supporting the various EMR applications that VNA uses and contributes to the successful adoption of these systems.

PROGRAM DIRECTOR, NAVIGATOR SERVICES: AURORA - FULL-TIME - HEALTH CENTER

Candidates must be willing to travel.

Education:  Bachelor’s degree required preferred.

Knowledge/Skills: Supervisory and management skills required.  Knowledge and ability to establish professional relationships, establish goals and prepare for future needs of the program.  Ability to communicate effectively and openly with all staff.

Type/Years Experience: At least one year of relevant experience.

Job Summary:  Responsible for the day-to-day management of the Navigator Services Teams, including those associated with the HRSA Outreach & Education (O&E), In-Person Counselor, and Federal Navigator grants.  This position oversees all regions in which the organization provides services and assesses, plans, coordinates, implements, and evaluates all program and grant management activities. Provides direct supervision of Team Leads and Navigators. 

REGISTERED NURSE - PHONE TRIAGE:  AURORA - FULL-TIME - HEALTH CENTER

Education: Bachelor’s degree in nursing preferred.

Knowledge/Skills:  Bilingual in Spanish a plus. Comprehensive knowledge and skill in current nursing practice; concept of multidisciplinary team approach to nursing care; knowledge of professional limitation and scope of practice within the Illinois Nursing Act.  Able to work effectively and efficiently in the EMR

Type/Years Experience:  Registered Nurse in the State of Illinois required. Minimum one year experience as RN, preferably in physician office setting needed. Previous experience in community health is preferred.  Education, training and/or experience in child health, immunizations and/or women’s health (including family planning and STDs) preferred.

Job Summary: The Phone Triage Nurse is responsible for patient related communication for VNA sites. This includes answering and returning patient related phone calls, renewing prescriptions, scheduling tests and appointments and other related activities.

FLOW COORDINATOR:  ELGIN - FULL-TIME - HEALTH CENTER

Education:  High school education. Some college preferred.

Knowledge/Skills:  Bilingual in English/Spanish oral and written word preferred. Basic computer skills required. Basic word processing and spreadsheet skills (Word/Excel). Excellent communication, multi-tasking and organizational skills.

Type/Years Experience:  Three to five years of experience in a medical center setting in a position interacting with patients, Providers and clinical staff. Similar experience will be considered. Prior experience working in a fast paced environment required.

Job Summary:  The Flow Coordinator facilitates the movement of patients through the Health Center in a safe and efficient manner.  This position will communicate effectively with Providers, Team Leaders, Clinical Staff, and Registration Staff in order to ensure patients receive prompt care.  Has the ability to multitask in stressful situations in order to effectively manage patient flow through Health Center services.  Ensures Health Center cycle time goals are attained.

BILINGUAL PATIENT CARE ASSOCIATE (CNA):  AURORA - PRN

Education: Requires high school diploma or G.E.D. Provides proof of certification from approved Certified Nurse Assistant program.

Knowledge/Skills:  Ability to perform tasks involving physical activity which includes heavy lifting and extensive bending and standing. Bilingual in English/Spanish required. Ability to type and basic computer skills. Competencies desired include finger sticks, use of a glucometer and hemocue, urine dipstick testing, ability to use a manual blood pressure cuff and a manual scale. Phlebotomy certification a plus.

Type/Years Experience:  Minimum 1 year CNA experience preferred

Job Summary: Performs basic screening procedures for primary care clinic as assigned. The Patient Care Associate also performs other tasks as assigned to assist in the well functioning of on and off site clinics.

DIRECTOR OF PAYROLL:  AURORA - FULL-TIME

Education:  Accounting degree required.  Certified Payroll Professional preferred.

Knowledge/Skills:  Intermediate or advanced Excel skills. Computerized payroll and accounting systems experience, Paylocity, Mysis/Alscripts and SAGE preferred.  Strong Excel Skills to include working with pivot tables and vlookups required.

Type/Years Experience:  Payroll experience required.  Two years of accounting or general ledger experience.  Experience in health care desirable.

Job Summary:  Professional position providing technical accounting support in the Finance Department to meet financial and statistical reporting requirements and processing payroll and payroll-related requirements.

CERTIFIED NURSING ASSISTANT - HOME HEALTH & HOSPICE:  AURORA - PRN

Education:  High School Diploma or equivalent required.  Registration on the Department Nurse Aide Registry & certificate of completion from accepted CNA training program required. 

Knowledge/Skills:  Bilingual in English/Spanish preferred.  Completion of formal phlebotomy program and hospice certification preferred.  Must have current driver’s license and automobile liability insurance for travel.

Type/Years Experience:  Prior CNA experience preferred.

Job Summary:  A Certified Nurses Aide (CNA) performs specific patient tasks assigned by a Registered Nurse (RN). Supportive services of the CNA assist in meeting the personal and palliative care needs of the patient and provide emotional comfort to the patient and their family members.  Specifically for the Home Health patients, the CNA shall assist the patient toward safe independent living.  The CNA works under the direction of an RN. The CNA is a member of the Hospice Interdisciplinary Team (IDT).

OPTICIAN:  AURORA - PART-TIME/FULL-TIME & PRN

Education:  High school diploma or equivalent required, college degree preferred. 

Knowledge/Skills:  Bilingual in English/Spanish required.  Experience with electronic health records systems and software preferred.  Must be able to read and interpret clinical data.  Proficiency in Microsoft applications, including Word and Excel.

Type/Years Experience:  Previous experience working in a community health organization preferred.  Maintains current CPR certification. 

Job Summary:  The Optician designs, fits and dispenses corrective lenses for the correction of a patient’s vision.

DOULA:  AURORA/ELGIN - FULL-TIME

Education:  High School Diploma required.

Knowledge/Skills:  Bilingual in English/Spanish required. Creative/knowledgeable about community resources.  Demonstrates motivation and ability to learn and apply basic parenting skills.  Available to participate after hours on an "on-call" basis.  Must have current driver’s license and automobile liability insurance for travel.  Non-judgmental and culturally competent.  Demonstrates good writing and organizational skills.  Strong interpersonal skills/ability to relate to people.

Type/Years Experience:  Some computer skills required.  Experience in successfully working with infants and young children/parents.

Job Summary: The Doula position is a trained birth assistant who provides physical and emotional support during the woman's third trimester, during labor, and postpartum. The Doula works collaboratively with the Family Support Worker to ensure quality, comprehensive care.

MEDICAL VEHICLE DRIVER:  AURORA - PRN

This position is scheduled on an as needed basis.

Education:  High school diploma or equivalent

Knowledge/Skills:  Bilingual in Spanish preferred.  Minimum of one year experience preferred.

Type/Years Experience:  Class A or B CDL License required.  Clean driving record and proof of automobile insurance.

Job Summary:   The Medical Van Driver operates and maintains the mobile medical vehicle and performs other tasks as assigned to assist in well functioning of mobile medical vehicle clinics.

REGISTRATION/EXIT - CLINIC SUPPORT STAFF: AURORA - FULL-TIME

Education:  H.S. Diploma or G.E.D.  Some office technology courses desired.

Knowledge/Skills:  Bilingual in English/Spanish required. Ability to alphabetize and add/subtract/multiply and divide necessary and to effectively communicate verbally and in writing. Manual dexterity sufficient to perform primary functions. Tactile, visual and olfactory senses intact. Ability to deal with high levels of stress.

Type/Years Experience:  Minimum of one year experience in medical office preferred.

Job Summary:   Employee performs basic demographic and visit screening procedures; clerical and record keeping duties to assist in well functioning, customer focused clinics.

MAMMOGRAPHY TECHNOLOGIST:  AURORA - PRN

Education:  AMA Approved program in Radiological Technology

Knowledge/Skills:  Digital mammography and bone densitometry skills required.

Type/Years Experience:  ARRT and iEMA licensure required.  BLS certification and MRT license preferred.  Two years experience as a staff technologist preferred.  Mammography certification preferred.  Bilingual in English/Spanish preferred.

Job Summary:  Performs breast-imaging procedures to include screening and diagnostic mammography. Must have working knowledge of the Radiology Information System and PACS. This position requires full range body motion, including handling and lifting patients when necessary. Able to work evenings and Saturdays as required.


EMPLOYEE SERVICES & CREDENTIALING SPECIALIST:  AURORA - FULL-TIME

Education:  Bachelor’s degree preferred.

Knowledge/Skills:  Detail oriented with time management skills.  Ability to work in a fast pace and stressful work environment.  Knowledge and proficiency in Microsoft applications.  Ability to communicate and cooperate effectively with all staff and community contacts.  Must have current driver’s license and automobile liability insurance for travel.

Type/Years Experience:  Three years of prior Human Resources experience.

Job Summary:  The Employee Services & Credentialing Specialist works closely with the Director of Employee Services to efficiently and effectively onboard employees and maintains the integrity of employee records.  This position also has responsibility to provide day-to-day HR support covering a wide range of topics, from providing employees and managers support with employee relations concerns and answering general questions regarding policies company policies. Responsibilities are carried out in a positive manner with a focus on creating a work environment where employees feel respected, cared for and well-informed.  Relies on extensive work experience and judgment to plan and accomplish goals. Performs a variety of tasks while maintaining confidentiality and integrity of VNA.
